CHFT has now launched its Rapid Access Arrhythmia Clinic for patients with AF, Atrial Flutter or SVT. Patients will be seen within 2 weeks in a one stop clinic.  Read more here...

Changes to products available on prescription

Calderdale Bladder and Bowel Service 

The Continence Advisory Service has recently been rebranded. It caters for adults and children over the age of 5 years. Referrals can be made via GPs and other health professionals. Patient self-referrals are also accepted.   Find out more here...

Electronic Reporting of Deaths to the Coroner 

The Coroners office have changed their reporting procedure recently. Those referring a death are now required to complete an electronic referral form which is sent via secure email to the Coroners Office. This is an interim procedure pending the introduction of an online portal.  Read more here...

The District Nurse team have shared guidance regarding local pathways for lower limb problems including referral criteria for the Lymphoedema specialist team, Leg ulcer/District nurse clinic, Vascular Clinic and Podiatry Department.  Read more here...

Noah's Ark Centre for Counselling and Psychotherapy Services 

Noah's Ark Centre's counselling and psychotherapy services are now open to referrals from new clients. Charges have been changed to a donation basis. More details and a downloadable leaflet can be found  here.

NHSE Patient Referral Leaflet 

NHS England have produced a  patient referral leaflet  which describes what happens when a patient has been referred by a GP to see a specialist. They are supplying 200 leaflets per practice.
